During your first appointment, we will perform a comprehensive medical and dental health history with a thorough examination and charting to evaluate for bone loss, loose teeth, gum health, occlusion (bite), oral cancer screening, TMJ, and any signs of periodontal disease. Using state of the art digital radiographs, together with your needs in mind, a treatment plan is developed which will be explained to you in detail. We will also answer any questions or concerns that you may have.

 

Please assist us by providing the following information at the time of your initial visit to the office:

1) Referral slip and x-rays, if applicable, from your referring dentist.

2) A list of medications you are currently taking.

3) Dental insurance information and card.

Please notify the office if you have a medical condition or concern prior to this examination or surgery (e.g. artificial heart valves or joints that require pre-medication, severe diabetes, currently taking blood thinners, or uncontrolled hypertension.)

Important: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
